What to bring to consultation

  • Insurance card (primary, and secondary if you have one)
  • Your child’s date of birth and any diagnosis / referral paperwork you already have
  • A short list of your biggest hopes or concerns (even three bullets is enough)
  • Questions about schedule, wait time, or whether we are a fit
